Dr. Ward’s extensive educational credentials include a five-year residency in Otolaryngology-Head & Neck Surgery followed by completion of a Facial Plastic Surgery Fellowship culminating in board certification in facial plastic surgery. This highly coveted training provided extensive training in the area of complex cosmetic and reconstructive surgery of the face and provided the foundation for him to develop his practice focusing on rhinoplasty, revision rhinoplasty, facelift, blepharoplasty, and cosmetic injectables.

Dr. Robert Rodrigues is a board-trained plastic surgeon with over 20 years of experience in aesthetic and reconstructive procedures.
He completed his undergraduate studies at Carroll College and earned his medical degree from the University of Colorado Health Sciences Center, followed by plastic surgery training at Case Western Reserve University and the University of Michigan and an advanced fellowship at the Mayo Clinic. He specializes in facial rejuvenation, breast enhancement, body contouring, and fat transfer, approaching each patient with precision, artistry, and individualized care. Known for his meticulous technique, he emphasizes patient safety, clear communication, and customized plans while staying at the forefront of advanced aesthetic methods to ensure optimal, natural results.

With over 16 years of experience as a nurse, I’ve spent the last 6 years specializing in aesthetics. My passion lies in helping others feel confident and beautiful. I particularly enjoy performing lip fillers and our signature tox treatments.

Lisa (Elisabeth) Folkman
Lisa Folkman brings decades of experience in healthcare to her role as Director of Operations at the Ward Institute, combining clinical expertise, operational leadership, and a deep commitment to patient care.
She began her career as a Certified Medical Assistant and later advanced her education, building a strong foundation across a range of specialties including cardiology, vascular surgery, urgent care, family medicine, and oncology. Her professional background includes key roles at respected institutions such as the University of Utah, Primary Children’s Hospital, and the Huntsman Cancer Institute.
Lisa developed a long-standing passion for aesthetics early in her career when she was a Cosmetologist. Today, she is a strong advocate for her patients, deeply invested in their safety, comfort, and overall well-being. She is especially passionate about the life-changing impact of cosmetic procedures and is committed to ensuring every patient feels supported, informed, and empowered throughout their journey.
In addition to her healthcare expertise, Lisa is the founder of Wicked Good Salsa, a growing small business. At Ward Institute, she oversees the full scope of operations—including HR, payroll, financial planning, surgical coordination, and staff management—with a focus on integrity, compassion, and excellence.
Originally from out of state, Lisa now proudly calls Utah home. She’s an avid animal lover with a house full of pets and finds her greatest joy in her four grown children and six cherished grandchildren.
Fun Facts: Lisa is a devoted Detroit Red Wings and Utah Mammoth fan, and is passionate about hockey. She also cheers for the Michigan Wolverines, Utah Utes, and Detroit Lions football teams—and this girl loves her Tigers! (she is also a Red Sox fan) but don’t tell.

With over 3 years of experience and a passion for healthy, radiant skin, I am dedicated to helping you achieve your unique skincare goals.
I draw upon my comprehensive training and hands on experience to provide safe, effective, and personalized treatments that are enhanced by your at home skin care routine.
My expertise lies in beauty treatments, and I utilize advanced techniques and technologies to address acne scarring to signs of aging and hyperpigmentation. I also have extensive expertise in laser hair removal, microneedling, chemical peels, and laser treatments.

Jessica Hyde
I have been a part of Form Spa and Ward MD for five years! First, I was a patient of Dr. Ward for my rhinoplasty in 2018 and became a member.
I loved attending the events and getting to know the staff. I loved using the FormRx products, getting HydraFacials and of course Botox. In the Fall of 2019, I was hired to work at the front desk and moved to the administrative side in January 2020. During Covid, I worked from home managing the membership program and have been managing it since!
I am super fortunate to work from home with my two littles. I tell people that I enjoy being able to use my adult brain in between playing and accomplishing all my mom-duty responsibilities.

Christian is an IT graduate with a strong background in technology and a creative foundation from earlier years spent in musical theater.
With over a decade of experience in customer service, he has developed a deep understanding of client needs. In addition to working in tech, Christian also works part-time as a certified sports nutrition specialist, focusing on low-carb and keto lifestyles. Bringing together technical expertise, people skills, and a passion for health and wellness, he offers a well-rounded, thoughtful approach to every professional and personal endeavor.
Fun fact: Christian is also a proud pet parent to two lovable dogs, Butter and Prince.
